Wild swimming brings a water-quality warning

A randomised controlled study of bathing in Hungarian rivers and lakes found that swimmers reported skin problems more often than people who stayed on shore. Gastrointestinal illness was uncommon overall, but the risk rose when swimmers were exposed to higher concentrations of E. coli and other faecal-pollution indicators.
The study included 2,368 participants at two rivers and two lake-like bathing sites. People were randomly assigned either to enter the water for ten minutes, immersing their face at least three times, or to remain out of the water. Researchers sampled the water through the trial and asked about symptoms a week later.
Skin complaints — rash, ulcer or itching — were reported by 2.7% of swimmers and 1.3% of non-swimmers. That is roughly twice as common among swimmers. About 2.1% of swimmers and 1.9% of non-swimmers reported gastrointestinal illness, so simply entering the water was not statistically linked to stomach illness in the overall comparison.
The more specific finding concerned contamination. Among swimmers, higher E. coli and somatic coliphage concentrations were associated with greater gastrointestinal risk. The study did not find the same relationship for intestinal enterococci. The authors argue that freshwater monitoring should continue to include E. coli, while the cause of skin irritation remains uncertain.
The result has limits. Each site contributed one trial day, some participants swam elsewhere, and the work took place in Hungary. The researchers point to a similar German study and say the findings may apply to the UK, but local water, weather and behaviour can differ.
The practical message is not ‘never swim outdoors’. It is to treat water-quality information as part of the decision, avoid swallowing freshwater, and take skin symptoms seriously. A clean-looking lake can still contain microbes or irritants that are invisible.
For public-health policy, the study is a signal about what should be measured. For swimmers, it is a reminder that risk depends on exposure and water conditions, not just on whether a site looks safe.
